Comparison of the Demographic Data of Total Laparoscopic Hysterectomy and Vaginal Hysterectomy Cases

INTRODUCTION: The study aims to investigate whether Total Laparoscopic Hysterectomy (TLH) can be an alternative method to Vaginal Hysterectomy (VH) in cases operated with benign indications. MATERIAL-METHOD: A total of 109 patients who underwent TLH and 99 patients who underwent VH for benign indications were evaluated retrospectively. Patients' ages, parity numbers, hysterectomy indications, pre-postoperative hemoglobin/hematocrit (Hb/Hct) values, operation times, uterine volumes, hospital stays, intra-postoperative complications, and post-operative pathology results were recorded. In addition, patients who underwent TLH were evaluated by dividing them into two different groups using 2D and 3D methods. RESULTS: While there were no statistically significant differences between the TLH and VH patient groups in terms of parity numbers, postoperative Hb/Hct decreases, hospitalization durations, and intrapostoperative complications (p>0.05), there were significant differences in terms of age, operative times and uterine volumes (p<0.05). The most common hysterectomy indications in the TLH group were myoma uteri, while the most common indications in the VH group were descensus. CONCLUSION: The laparoscopic method is a safe and effective method when performed by experienced surgeons. Although the operative results for TLH are comparable to Total Abdominal Hysterectomy (TAH) and VH procedures, the VH procedure still maintains its place as the gold standard method for benign hysterectomy operations. In addition, although no statistically significant difference was found in terms of complication rates, postoperative Hb/Hct decreases and hospital stays, it was seen that the complication rates, postoperative Hb/Hct decreases, and hospital stays were numerically less in TLH cases. This situation showed us that TLH could be an alternative or even a replacement procedure for VH in hysterectomy operations performed for benign reasons, with the increasing prevalence of laparoscopy in the future and the increasing experience of surgeons in parallel.
