Innovative Treatment for Degenerated Bioprosthetic Tricuspid Valves: Valve-in-Valve Approach

Transcatheter valve-in-valve (TVIV) implantation has emerged as a promising therapeutic option for patients with degenerated tricuspid bioprosthetic valves, particularly those deemed high-risk for traditional surgical approaches. Over time, degeneration of tricuspid bioprosthetic valves can lead to significant hemodynamic impairment, adversely affecting patients' quality of life. Conventional surgical interventions, while effective, are associated with substantial complications and prolonged recovery periods, especially in patients at elevated risk for cardiac surgery. In this report, we present the case of a patient who previously underwent bioprosthetic valve replacement for functional tricuspid insufficiency and presented with valve degeneration 10 years post-surgery. The patient was successfully treated with a TVIV procedure using a 29 mm Myval transcatheter heart valve (Meril Life Sciences, Vapi, Gujarat, India). Following the procedure, no complications were observed, and valve function significantly improved. This case illustrates the potential of TVIV as a viable alternative in managing degenerative bioprosthetic valve disease. It offers a safe and effective treatment modality, particularly for high-risk patients, thus avoiding the need for repeat surgical interventions. The favorable outcomes observed in this case further emphasize the growing role of TVIV in the treatment of bioprosthetic valve pathologies and its potential to improve outcomes in high-risk patient populations
